\nPosition Summary: We are seeking an experienced and compassionate Program Director to lead a well-established Adult Day Care program dedicated to delivering high-quality, person-centered services grounded in the social model of care. This leadership role focuses on fostering a supportive and engaging environment that promotes social interaction, well-being, and independence for program participants.
\nKey Responsibilities:
\nDevelop, implement, and oversee programs that encourage social engagement and community participation.
\nMonitor program outcomes and apply data-driven improvements.
\nEstablish and maintain policies aligned with the social model of care.
\nLead, mentor, and supervise program staff to promote teamwork and professional development.
\nConduct staff evaluations and coordinate ongoing training initiatives.
\nCultivate partnerships with community organizations, healthcare providers, and families.
\nRepresent the program at external events to raise awareness and support.
\nManage budgets and allocate resources to maintain program sustainability.
\nIdentify and pursue funding opportunities, including grants.
\nAnalyze and report on program data to guide strategic planning.
\nAdvocate for participants’ needs within the broader community and healthcare systems.
\nStay informed of industry best practices and trends related to adult day care services.
\nQualifications:
\nBachelor’s degree in a healthcare or related field preferred.
\nExperience managing programs in adult day care or social services preferred.
\nStrong knowledge of the social model of care and relevant regulatory requirements.
\nExcellent leadership, communication, and interpersonal skills.
\nDemonstrated ability to collaborate effectively with diverse populations and stakeholders.
\nProficiency in data management and reporting tools.
