Job Description

Medical Adult Day Care Transportation Coordinator

Job Details:
  • Responsible for maintaining a schedule of transportation to and from the center which includes keeping meticulous records of the pick-up and drop-off of participants.
  • Serves as the primary contact for transportation-related inquiries and concerns.
  • Monitors and responds to daily route issues, including delays, cancellations, and emergencies
  • Assists the participant out of the home and into and out of the vehicle, as necessary.
  • Reports to program director immediately if a participant does not appear for previously scheduled transportation.
  • Makes effort to determine if the participant is helpless and unable to answer the door or the phone or has gone away for the day.
  • Maintains a schedule of maintenance of the vehicle, including documented safety checks of the safety devices.
  • Must be physically able of performing the required duties and of assisting frail, elderly, and disabled adults, as necessary.
  • Able to read and write English, fluently.
  • Assists with client transportation services as needed.
  • Must pass all Driver Training requirements assigned.
  • Must provide the center with a health examination, signed by a physician and a TB clearance which is not more than 12 months old prior to employment or within 7 days of employment and shall provide the center a current certificate of CPR and First Aide training, or shall complete training within six months of employment.
  • Must pass a criminal background check as required and may not have criminal convictions or criminal history that indicates behavior that is potentially harmful to participants as evidenced through a criminal history records check.
  • Must be able to successfully complete the company’s physical job demands.
  • Must be able to pass DOT physical.
  • Ensure other drivers have completed trainings as assigned.
  • Completes other duties as assigned.


Education and Work Experience:

  • High School education or GED.
  • Must be at least 18 years of age.
  • Must have experience in transporting patients.
